Why is Dilation Needed for Eye Exams?

Comprehensive eye exams are about more than great vision. Regardless of age, a majority of eye problems arise without a patient’s knowledge, and many conditions, such as retinal detachment, glaucoma, and macular degeneration, can be treated successfully when caught early. In addition to checking for prescription changes, Drs. Scott and Staci Ecenbarger also examine the back of the eyes to confirm the retina and macula are healthy and don’t show signs of damage or disease.

When your pupil is small, the doctor’s view of the optic nerve and macula is limited. Prior to advanced diagnostic testing, the only way to examine the entire retina, optic nerve, and macula, was to dilate the pupil (or make it larger) by placing special drops into the eyes. While dilation enables the doctor to see 100% of the retina, they can only see about ⅛ at a time. It also requires 15-30 minutes for the drops to take effect after being administered, and patients often experience light sensitivity and difficulty focusing for 3-5 hours after their appointment.

Premier diagnostic testing is quick, easy, convenient, and also accurate. Drs. Scott and Staci can use this advanced testing to get a detailed view of the retina and macula, allowing them to spot any potential issues with ease. They can also save images for future reference, providing peace of mind knowing your eye health is being monitored closely.

Which is Better?

While Drs. Scott & Staci recommend the Premier Diagnostic Package whenever possible, both are great! The Optomap diagnostic test provides an overview of the retina in a single photo, offers a more detailed view than can be achieved by dilation, and provides a permanent record for in the patient’s medical file. iWellness OCT provides a cross-sectional view of the macula, which is particularly helpful in the early detection of macular degeneration and provides an early-warning system for glaucoma.

For patients with existing medical conditions, a significant family history, or too long a gap since their last dilation, dilation may still be necessary. When dilation is required, the benefits of diagnostic testing still apply, including the ability to save images to a patient’s file, to zoom in for a more detailed view, and to share pictures with patients to review findings. Therefore, we offer the opportunity to do both. The permanently recorded images also enable the doctor to monitor changes and compare your eye health year-over-year.

Are online eye tests any good?

Have you considered getting online eye tests? The idea of being able to get an eyeglass prescription and buy glasses without a trip to the eye doctor may sound appealing. Before ditching the traditional eye exams, there are a few things you need to know!

Online Eye Tests

The most important thing to know about online eye tests is they do not evaluate the health of your eyes. Even if they are called “online eye exams,” these exams only measure your visual acuity and refractive error. Some online eye tests can check for contrast sensitivity and color blindness. However, none of this can tell the health of your eyes.

The only way to know the complete health of your eyes is through eye exams with your doctor. During an eye exam, your doctor can detect vision-threatening conditions such as cataracts, glaucoma, or macular degeneration. Early detection of these conditions can prevent vision loss and blindness.

Know The Risks:

  • Online eye exams cannot detect eye diseases
  • Improper testing can occur due to error or misreading instructions
  • Higher chance of getting incorrect prescription due to self-administered the eye test
  • If you think the prescription is incorrect, your only option is to pay again and retake the test
  • An eyecare professional is not present to answer questions or concerns

Validation of Online Eye Tests

The results of online eye tests have not been guaranteed to be accurate measures of your prescription. Due to this being relatively new technology, there have not been enough studies to determine the reliability and validity of online eye tests.

Additionally, many online eye tests say their technology is suitable only for people between the ages 18 and 40 who are in good health. The limitations of the eye test raise concerns to the overall validity of the test. For these reasons, we do not recommend them as your sole option for your receiving your prescription.

The best way to ensure your eyes are healthy, you receive the correct prescription, and get answers to all your questions is through face-to-face eye exams with your eye doctor. Vision Development and Children

Childhood is a critical time for vision development. Nearly 80% of what a child learns in school is presented visually. Arguably making vision the most important of the five senses. Visual skills start developing during pregnancy and continue to evolve and develop as a child grows. Undetected vision problems can cause developmental and educational delays in children.

Infant Vision Development

Your infant’s vision starts developing during pregnancy. It is crucial that toxins are not consumed during pregnancy as they can cause serious vision problems. At birth, babies only see black, white, and shades of gray. Infants are unable to focus on objects for several months and can only see the outline of objects.

As infants grow, they can distinguish between high contrast colors. By six months your child can see color, has sharper vision, and has begun developing hand-eye coordination skills. Schedule your child’s first eye exam at six months to make sure their eyes are healthy and on the right developmental track. Detection of eye health issues and vision problems at this stage in development can help to ensure your child does not experience setbacks in learning and growth.

When your infant begins to crawl and potentially walk they are learning to coordinate their body movements and their vision. Over time, your child will become better at judging distances. However, this is also a time when your child may grow more injury prone because they are exploring their environment. Bumps, bruises, eye injuries, and other injuries can occur which is why it is so vital to ensure that your infant’s vision is on track to prevent these injuries as much as possible.

Early Childhood Vision Development

During these years your child will be growing, developing, and improving their visual skills. It is recommended to schedule your child eye exam at three years old. Even if you don’t think your child has vision problems, your child is growing and changing. A comprehensive eye exam before your child enters school provides enough time to catch and correct any vision problems.

They are discovering how to integrate their vision and body position to complete new tasks. They learn this through playing games, throwing a ball, and riding a bike. Children are also working on developing their fine motor skills. The primary way preschool age children are learning this is through writing their name and the alphabet.

Between the ages of 3 to 6 is when you, as a parent, may begin to notice signs of a vision problem. If your child complains about headaches or tired eyes, this could potentially be due to a vision problem. Signs of vision problems include squinting, tilting the head, frequently rubbing eyes, and closing one eye to see. Additionally, look for sitting too close to a tv, holding a book too close, or avoiding activities that require near or distance vision. Some of these activities include coloring, reading, playing ball, or tag if you notice these signs in your child schedule an eye exam as soon as possible. Correct their vision before any learning is delayed!

Screen Time and Children

Screen time is the amount of time a person spends staring at digital displays including computers, tablets, smartphones, and TVs. In our modern and technology-focused world children are spending time on digital displays for educational and recreational purposes. Children who spend several hours on digital devices are at risk of developing vision-related problems.

Average Time Children Spend On Digital Devices

According to the Vision Council, 72% of American parents report their children regularly spend more than two hours on screens per day. It is likely that children spend significantly more time on screens than their parents think. Common Sense Media reports that children under age eight spend more than two hours a day with screen media. For 8 to 10-year-olds screen time triples to six hours per day. Kids in middle school and high school spend up to nine hours per day looking at digital displays.

Risks of Screen Time

Too much screen time can be dangerous for anyone’s eyes, children included. Screens emit a broad spectrum of visible light. While most of these light rays are harmless, blue light is a high-energy visible light that can cause damage to your eyes. Blue light has shorter wavelengths and higher energy causing harm to the retina over time. Overexposure to blue light can cause:

  • Difficulty sleeping
  • Headaches
  • Neck/shoulder pain
  • Eye strain
  • Reduced attention span
  • Poor behavior
  • Irritability

Computer Vision Syndrome

Computer vision syndrome is a condition caused by visual stress. Symptoms include tired eyes, dry eyes, headache, and fatigue.

Unhealthy Posture

Your body naturally slouches inwards when on digital devices. Your back and shoulders round, your head tilts back, and your chin justs forward. This reaction to digital devices is called “turtling” and can cause neck, back, and shoulder pain.

How To Protect Your Child’s Eyes

It is clear digital devices will not be going away anytime soon. Therefore it is essential to ensure you are doing everything you can to protect your children’s eyes from digital screens. One way you can do this is by limiting screen time for your children while at home. You can also apply blue light filters or download blue light filtering apps to all digital devices. If your child wears prescription glasses, ask us about add blue light blocking to their lenses during your next appointment.

Nighttime Use

The largest source of blue light is our sun, which tells our brain when to be awake or sleep. The high use of digital devices emitting blue light may disrupt your natural circadian rhythm (sleep cycle) by miscommunicating the time of day and if you should be awake or asleep. Stop digital device time two hours before usual bedtime to ensure your child’s sleep schedule affected by blue light.

Corrective Eye Surgery Basics

Corrective eye surgery can address a number of eye problems. Most people have heard of laser eye surgery to eliminate the need for prescription glasses. As the procedure has gotten easier and less expensive, more people are opting for things like LASIK and telling their friends about the results.

LASIK

LASIK is the most popular refractive surgery. Because healing time is fast, many people check to see if they are good candidates for LASIK before considering other options. With LASIK, a flap is made on the front of the cornea. A laser is then used to remove some corneal tissue and smooth irregularities in the surface of the lens. The cornea can be made flatter or more rounded to cure nearsightedness or farsightedness, as well as astigmatism.

Photorefractive Keratectomy (PRK)

Photorefractive Keratectomy (PRK) was the first laser eye surgery that became widely available for vision correction. It may help people who are nearsighted, farsighted, or have astigmatism. PRK is similar to LASIK, but the main difference is that the procedure does not involve cutting a flap on the surface of the eye to resurface the lens. With PRK, the epithelium covering on the eye is essentially buffed away, and the surface of the cornea is reshaped using an excimer laser. This means the protective covering on the front of the eye has to heal which creates a longer healing process for PRK than some other procedures.

LASEK

LASEK is a corrective eye surgery that can also address myopia, hyperopia, and astigmatism. LASEK reshapes the front of the cornea with a specialized laser procedure, eliminating the need for glasses in some patients. The thin covering on the front of the eye is not entirely removed like with PRK. Instead, an even thinner flap than what LASIK uses is created and then replaced after the procedure to act as a natural bandage. Some people who cannot get LASIK are instead candidates for LASEK because it can be performed on someone with a thinner cornea.

Cataract Surgery

Cataract surgery is also considered corrective eye surgery. For this type of surgery, the cloudy lens of an eye with a cataract is removed and an artificial lens is replaced. Vision is then made clear again, which often reduces the patient’s need for glasses. The procedure is very common and considered one of the safest and most effective corrective eye surgeries. As our population ages, instances of cataracts surgery are on the rise and continue to provide great results.

The different kinds of corrective eye surgeries have their benefits and possible side effects. Make sure that you consult a qualified eye doctor if you have questions about these procedures.

The foods you eat and the dietary supplements you take affect your overall health and the health of your eyes. Nutrition and your eyes are linked together and can help prevent certain eye diseases along with other health problems.

Healthy Foods

Choosing healthy foods improves your overall health as well as your eye health. Dark green or brightly colored fruits and vegetables are essential parts of a healthy diet. These fruits and vegetables may also help to reduce the risks of developing eye diseases. Sugars and white flours may increase your risk of age-related eye disease, instead, opt for whole grains which do not have the same risks. Healthy fats containing omega-3 essential fatty acids are critical to your diet. These healthy fats can help prevent dry eyes and cataracts.

Hydration

Staying hydrated is essential to the health of your eyes. Drink plenty of water every day! We also recommend choosing healthy beverages and avoiding high sugar beverages. Proper hydration is linked to the reduction of dry eye symptoms.

Nutrients

Nutrients are an essential part of a healthy diet. These nutrients can be found in foods but can also be taken in supplements to ensure you are receiving the proper amount in your diet. Consult with your primary care provider before taking any dietary supplements. Here are a few nutrients that may have a link to eye health:

  • Vitamin A: may protect against night blindness and dry eyes
  • Omega 3 fatty acids: may prevent macular degeneration and dry eyes
  • Vitamin C: may reduce risks of cataracts and macular degeneration
  • Vitamin D: may reduce risks of macular degeneration
  • Zinc: may reduce risks of night blindness
  • Vitamin E: may reduce the risk of advanced macular degeneration

Aging Eyes

As you age, it is essential to consider all factors that could affect the overall health of your eyes. Not only should you adopt a healthy diet, but you can also do several other things to protect your eyes. One way to protect your eyes is to avoid overexposure to ultraviolet rays, which includes wearing sunglasses outdoors and staying away from tanning beds. Now is the time to quit smoking, not only is smoking harmful to your overall health it also increased your risks for many eye diseases. Finally, ensure that you are getting annual eye exams to detect any eye diseases before they cause permanent vision loss.

Your Infant’s Eye Development

Parents are always curious about their developing children, and what they can expect for various stages of infancy. We’re happy to explain infant eye development, and to let parents know not only what to expect, but when they should bring their child in for an exam, and why this is important.

Birth to 4 Months

After being born, babies are suddenly able to see all kinds of new things! We know that babies don’t see very clearly for some time, so high-contrast books, toys, and images may get their attention better than others. After the first few months, eyes get better working together to focus on objects and aid hand-eye coordination. At about two months, most babies can focus on a parent’s face or nearby object. It’s common for children to appear to have wandering or crossing eyes. It doesn’t become a concern unless this happens frequently, or seems to get worse. By three months, your child can probably watch an object move through a room and keep their focus on it.

5–8 Months

As babies this age move more, interact with the world, and start to judge distance, their eyesight improves. It’s not until about five months that children are able to see and comprehend three dimensions, so they won’t begin to see depth until around now. Most babies crawl around eight months or so, but don’t worry if your baby isn’t crawling well right away. Children should be encouraged to take their time developing skills. Babies who crawl a lot usually learn to use their eyes together better than babies who were early walkers.

9–12 Months

Around this time, most babies are experimenting with pulling themselves up on furniture, or using someone’s fingers to keep themselves steady and talk a few aided steps. Don’t rush crawling, however. Babies who master crawling before learning to walk tend to have better hand-eye coordination. Additionally, babies at this stage are better able to judge distance and begin to throw and place items.

1–2 Years

Children between one and two years of age are usually highly interested in exploring their surroundings, interacting with objects, and occasionally making some pretty impressive messes! Motor skills will excel along with depth perception and their improving eyesight.

How Do I Help My Child’s Vision Development?

First, be sure to get your child an eye exam at about six months of age. Qualified pediatric optometrists are trained in using lights and lenses to understand what is going on inside your child’s eyes. Even without having the ability to relay information about how well they’re seeing, your baby’s eyes give clues about their development. If they show signs of an eye turn, focusing problem, or other issue that could impede development, it’s very important to seek treatment as soon as possible. An optometrist can diagnose these conditions and make sure your child is aided in visual development so that they can keep moving and learning freely!

Beyond getting an infant eye exam, children need another exam before starting school. In the meantime, look for any signs of vision trouble like bumping into things, not recognizing you from a distance, or eyes that seem not to focus in unison. If you have any questions, speak to an eye care professional. Otherwise, play with your baby and enjoy this time as they learn and grow.

Choosing the Right Eye Doctor

As is true with any health care, it’s important to find a doctor who is qualified to treat you. With eye care, you may be able to get an eye exam from a large number of practitioners. If you need to confirm a diagnosis, treat an eye disease, or have other unique needs like a pediatric or geriatric specialty, you will have to spend more time choosing the right eye doctor. No matter what your current vision situation is, it’s crucial to choose the right eye doctor for you and your family.

What is an Optometrist, Ophthalmologist, or Optician?

Many people use the term “eye doctor” to refer to the qualified individual who administers your eye exam and writes you a prescription for lenses to correct your refractive error. We know just how many distinctions there are between eye care professionals, however!

Most people who want to see an eye doctor will seek an optometrist. An optometrist holds a Doctor of Optometry (OD) degree and can examine eyes for vision or health ailments, visual acuity or refractive errors, and can prescribe eyeglasses or contact lenses. Some optometrists also provide additional services, like vision therapy and care plans for people with low vision.

An ophthalmologist is a medical doctor or osteopathic doctor who specializes in the eye. They can also perform eye exams, but they are more often used to diagnose and treat eye diseases, prescribe medications, and perform eye surgery. They are able to write prescriptions for eyeglasses and contacts, too.

Another term that people sometimes hear and may not know what the occupation entails is “optician.” An optician is not a doctor. These professionals are qualified to use prescriptions written by an eye doctor to help fit you for eyewear and explain use, care, and features of your glasses or contacts.

What kind of doctor should I see?

This depends on your vision and eye health. If you have not had problems with your eyes beyond typical blurriness at certain distances, and if you’re not concerned about risks of eye disease or more serious symptoms, generally you are fine to see an optometrist. If you aren’t sure and you see an optometrist, he or she will let you know if you need to seek treatment from an ophthalmologist.

How do I pick a good eye doctor?

This is up to you! Some people like to look at social media pages to see what is the personality of the practice. You may seek reviews from others online, or ask friends for suggestions. Others go based on convenience factors like when the office is open, how easily you can schedule an appointment, and proximity to your home or office. Some factors may require a visit. If you’re planning on getting new eyeglasses, most offices allow you to walk in any time that they are open to browse their selection of frames, and even to speak with an optician about specialty products and upgrades. This also gives you an idea of their customer service, friendliness, and general feel of the office.

Reduce Exposure to Blue Light, Inside and Out

Risks of blue light exposure have become a topic of discussion in recent years. More eyecare professionals and scientists are studying the effects of blue light to assess the effects of relying on blue light for so much of our lighting.

It’s hard to say for sure how much added blue light exposure may lead to additional cases of degenerative eye health problems like macular degeneration. Plus, there is some evidence that overexposure to high-energy blue light can cause side effects such as sleep disturbances, eye strain, and headaches. With this in mind, how can you reduce your exposure, inside and out?

Inside

  • Lower the brightness of your electronic screens, especially at night, but make sure you are not straining to see the words if you’re reading.
  • Take frequent breaks while working at a computer. Regardless of blue light exposure, Computer Vision Syndrome is another problem you may face, and taking breaks to relax your eyes will help.
  • Stop using your devices at least a couple hours before bed. Unplug and read a book instead, which will give your eyes something easy to focus on and help you wind down for good sleep.
  • Adjust your distance. Viewing a bright screen up close is fatiguing. Make sure to set your computer monitor and television at a comfortable distance, and don’t hold handheld devices too close to your eyes. You can adjust text size if reading a device at arm’s length is difficult.
  • Opt for non-glare lenses. Non-glare or anti-reflective treatments on your lenses will help reduce bright lights affecting your eyes.

Outside

  • Wear sunglasses. Sunglasses that are an appropriate darkness and block 99-100% of UVA-UVB rays are the best protection for your eyes.
  • Consider polarized lenses, too. Polarization cuts harsh glares that bounce off of water and other surfaces.
  • Don’t forget a hat! In especially bright outdoor settings a hat will help block bright light from above, even if your eyes are protected from the front.

Don’t Forget UV Protection This Winter

Most people think of sunglasses as summer gear, but in some ways, it’s even more important to wear sunglasses in certain winter conditions than in the warm summer months.

Thanks to public awareness campaigns about the risks of skin cancer, more people are aware that UV rays and intense sunlight are bad for our skin. Sunblock, clothing, and shade are regularly used to protect us. Do you know that UV rays are damaging to our eyes, too?

Prolonged UV exposure raises risks for:

  • Cataracts
  • Macular Degeneration
  • Pingueculae
  • Pterygia
  • Photokeratitis

In the warmer months, we think about protection from the sun because we expose so much skin and can experience sunburns when we’re not covered. In the winter, however, people are colder, and bundled up, so they don’t think of the sun as a risk. This is a problem because snow can reflect 80% of UV rays! That means you’re experiencing normal sunlight from being outside, and harsh glares from white surfaces.

The fatiguing effects of winter glares become apparent usually when doing outdoor activities like skiing, snowboarding, or sledding, but driving in bright wintry conditions can also be damaging and dangerous. It’s important to be protected year-round!

When looking for appropriate winter sunwear, remember:

  • Good sunglasses offer UV protection, so always be sure to check that they block 100% UVA/UVB rays.
  • Larger glasses are in style, which is great because the bigger lens protects more of your eyes and the skin around your eyes from UV damage. Look for a large lens and wraparound style.
  • Don’t worry about the color or darkness of the lens. Those things can vary while still providing adequate UV protection. Check the label for that information, or speak to an optician.
  • Remember, polarization is for more than fishermen! Especially in the winter months when ice and snow can cause harsh glares, polarization can be a big help.
  • Specialty eyewear exists for all of your favorite winter sports. Be sure your eyes are protected from the sun and safe during sports.
